Ideas about Faculty Course Surveys?

Hi Everyone,
We are trying to get an idea of what courses are available on our campus  that
have to do with civic engagement and service-learning. We are also interested in
what research professors might be undertaking that have the same elements. Does
anyone know of any useful tools in surveying instructors, departments, etc.? Is
there a standard form or manner in which to undertake this assessment process?
We can develop a survey, but I wonder if there is something that already exists
and if anyone has any ideas about implementation. We have a medical school, a
vet school, and a dental school that we would like to involve in this process as
well. Any advice would be appreciated. Thanks.
