There is an overwhelming amount of information available on the topic of
access to health care.  Here are some links that may be helpful, including
some actual survey instruments:

The National Service Resource Center's August 2003 newsletter is focused
on healthy communities and has some good resources:

There are also some national datasets that you might wish to tap for data
specific to communities in Maine:

You might search Medline (a database run by the National Library of
Medicine) for published research articles that are similar to what you
are trying to do and contact the authors for permission to review/use
their survey instruments:

Also, one of our board members, Tom O'Toole from Johns Hopkins University
School of Medicine, facilitated a service-learning project around this
issue that had an advocacy component.  You might want to contact him for a
copy of the survey tool he used:
