Medical Access Survey Tool

Was there any response to the request for sample surveys last week  by
Giordano in New Jersey?

Our campus is begining a community wide assessment, and access to
health care by underserved populations will be a very important
component.  If anyone has a sample survey or recent research in this
area, we would appreciate any assistance.
