Service Learning Survey

Hello - 
I am trying to put together a campus survey to assess my school's
involvement with Service Learning activities.  We are working on a plan to
implement service learning school-wide.  First we need to find out what
everyone is doing or not doing in service learning.  Has anyone out there
done a similar survey?   Have a sample they could send me?  Or any
guidance on the best way to do this survey?  I would appreciate any
comments.  Please respond directly to me.  Thank you for your help!
