I've attached a document that lists some references to look at actual survey
instruments. In addition, I would suggest that you browse our section
Assessment and Evaluation on our website
-http://www.gseis.ucla.edu/slc/assess.html    - read through the abstracts 
and
you might find some articles that address the community assessment aspect.  
I
would also check out the publications on Community-Campus Partnerships for
Health, to see if any of their materials might be of assistance.  Community
needs assessments are commonly done in the health education field. You can
access their website at http://futurehealth.ucsf.edu/ccph.html 

Finally, I'd suggest that you contact Marty Duckenfeld at Clemson University
(mbdck@CLEMSON.EDU) who's group there recently developed a community agency
survey that she might be able to share with you. 

Good luck.  This is a focus area that needs much attention. I hope that once
your project is completed, you will share the results with all of us!
