Evaluation tools

Hi,
I'm an undergraduate assisting a faculty member with evaluation and
assessment research in the hopes of developing a new, qualitative assessment
tool for the college as a whole.  I've been charged with writing up the
section on service learning, and I'm in search of examples.  If anyone who
uses specific surveys, portfolio organizing strategies, questions or
