survey of mathematics faculty members involved in service-learning

Dear SL colleagues,

The Mathematical Association of America (MAA) and the American Association
of Higher Education (AAHE) are collaborating on a project to advance
service-learning in mathematics education.

The main purpose of this very brief survey is to gather preliminary
information on people and schools involved with math-related service
learning projects. The results of the survey will be used to identify
contacts for more detailed follow-up and to provide to the mathematical
community a sense of the general range of such math-related service
learning projects.  The main output of this project will be a book to be
published jointly by the MAA and AAHE and distributed widely to
mathematical sciences departments at colleges and universities.

This survey is designed to be completed by facultys member who have
personally used service learning in a course. However, in the absence of
direct contact  with that faculty member, it may also be completed by
another faculty member who may have knowledge of such an undertaking
either at his/her own institution or at another institution.
