Manual/Handbook Evaluation Workbook

Kathy - We have several evaluation manuals and workbooks listed on our
website.  You may wish to browse through and see if any of those sound like

Hello,

I have heard of a workbook that was printed with different evaluations
forms that others have used that have worked well for them when they had
their manuals or handbooks evaluated by others.

I am looking for a printed evaluation form to go along with a manual I
am going to be putting together and sending out for others to evaluate.
