Faculty Question and Answer Sheet

I am trying to create a question and answer sheet that I can give to 
faculty members who may be interested in developing service learning 
courses.  Some of the information will be campus specific, but I am also 
looking for some ideas that can help faculty.  Does anyone have anything 
like that or can you suggest some good resources.
