Agency Criteria

Hi all,
        Just wondering if I could pick a few brains about a policy
issue.  As one of the procedures for our center we are planning to do
site visits at all the agencies our students may potentially serve at.
I was wondering what criteria other schools have set to help them asses
whether the risk at an agency is too high to send students to.  Does
anybody have some written criteria that they use that they could send me
a copy of?  Thanks in advance.
