Models that Work Competition 2000

For those of you who are involved with public health agencies or in the
health field, this competition might be of interest to you.  Here's a
chance to recognize some of your special programs or you community partners!

Models That Work:  2000 Competition Announcement
                                  

                           Call for Nominations

                           In November, the Models That Work (MTW) Campaign
will host its fourth national competition to honor and recognize
outstanding programs, or health systems, that
demonstrate innovation, quality and outcomes in primary health care. This
award is designed to acknowledge and promote the innovative models of
service delivery that create health, social and economic benefits for
vulnerable and underserved populations. 

                           Who May Apply?

                           Any organization that provides
community-oriented primary health care services to and underserved or
vulnerable population is eligible for the award. Honorees will receive
national recognition, an expense paid trip to Washington, D.C. for the
awards ceremony, technical assistance opportunities and honoraria for
promoting their innovations at national conferences and workshops.

                           Interested applicants should send their request
