service-learning/civic engagement survey instruments

Dear Members,

  I am working on a study to examine the relationship among binge drinking, depression and service-learning/civic engagement.

 I am wondering if anyone knows of survey instruments that ask specific, clear service-learning and civic engagement questions.  Any information provided will be greatly appreciated.
